To apply for a scholarship, a $10 Dues Payment is required. You will need these documents to submit the application: 1) Personal Statement 2) Letter of Good Standing from your pharmacy school 3) Pharmacy related work history. When completed, fill out the following form.
Scholarship Application
2 letters of recommendation are required. Recommenders should be non-family professional acquaintances and should submit their letters using the following form.
Important! Incomplete applications will NOT be considered. We recommend that you complete the application at least a couple of weeks before the application window closes and follow up with your recommenders to insure their letters are submitted. Don't wait until the last day! The most common reasons for incomplete applications are non-payment of dues and missing letters of recommendation. Applicants will be notified of the scholarship committee's decisions approximately 1 month after the application window closes.
